Gastric bypass surgery performed by remote control
June 26, 2021 · Uncategorized · (No comments)

Sunday, August 21, 2005

A robotic system at Stanford Medical Center was used to perform a laparoscopic gastric bypass surgery successfully with a theoretically similar rate of complications to that seen in standard operations. However, as there were only 10 people in the experimental group (and another 10 in the control group), this is not a statistically significant sample.

If this surgical procedure is as successful in large-scale studies, it may lead the way for the use of robotic surgery in even more delicate procedures, such as heart surgery. Note that this is not a fully automated system, as a human doctor controls the operation via remote control. Laparoscopic gastric bypass surgery is a treatment for obesity.

There were concerns that doctors, in the future, might only be trained in the remote control procedure. Ronald G. Latimer, M.D., of Santa Barbara, CA, warned “The fact that surgeons may have to open the patient or might actually need to revert to standard laparoscopic techniques demands that this basic training be a requirement before a robot is purchased. Robots do malfunction, so a backup system is imperative. We should not be seduced to buy this instrument to train surgeons if they are not able to do the primary operations themselves.”

There are precedents for just such a problem occurring. A previous “new technology”, the electrocardiogram (ECG), has lead to a lack of basic education on the older technology, the stethoscope. As a result, many heart conditions now go undiagnosed, especially in children and others who rarely undergo an ECG procedure.

By Nathan Wei

More than 80% of Americans will have back pain at some point in their life — whether from injury, overuse, or as a result of the degenerative process that accompanies aging. Chronic low back pain is frustrating, not only because of the severe pain but also because it can be difficult to effectively treat.

A particularly common cause of this pain is a herniated disk, also referred to as a “slipped disk.” The usual conventional medical solutions have been anti-inflammatory medications, muscle relaxants, injections, physical therapy, and surgery, all of which take an extended amount of time to take effect and which may not work for everyone. Recently, a novel type of treatment has been gaining favor. It is a non-surgical, non-invasive treatment for herniated disks called spinal decompression.

Your spine consists of bones, called vertebrae, each of which is stacked one upon the other. The vertebrae are separated from each other by rubbery discs that act as shock absorbers.

[youtube]http://www.youtube.com/watch?v=wt2gRQoZ53Y[/youtube]

The disks are constructed like hard donuts filled with a jelly-like material in the center. Over time, the strong fibrous cartilage (the donut shell) can weaken, allowing the jelly-like material (nucleus pulposus) to bulge. If its a mild bulge, it is not painful. However if the bulge extends out far enough, it can irritate nerves and lead to pain. But more seriously, a disc can herniate- explode through the shell causing disc material to pour out and press on spinal nerves. This can cause excruciating pain and significant nerve damage.

One of the first devices used for spinal decompression was approved by the FDA in 1995. Because spinal decompression requires special expertise and expensive equipment, few physicians have offered this treatment — but the procedure is becoming more commonly available.

Here’s how it workthe patient lies on a comfortable table made specifically for decompression, comfortably strapped down with a pelvis and torso harness that looks like a girdle. It is a high-tech traction device,” Dr. Wei explained. It works by slowly and comfortably creating traction by pulling and holding for one minute. Then, intermittently, it releases. It is believed that this creates a negative pressure, or a vacuum within the disk, which then sucks back the bulging or herniated disk material which was displaced,” he goes on to explain. With less pressure inside the disk, and therefore less on the spinal nerves, pain often diminishes and might even go away sometimes altogether. To maintain the benefit, however, numerous sessions may be required.

This theory is that this technique also allows nutrient and oxygen-rich fluid to travel to the area where there is less pressure, stimulating the healing process. Most patients either sleep or listen to a CD during the treatment, Dr. Wei added. Each session takes about 30 minutes and a typical treatment program may take between 20 to 30 sessions.

Critics contend that there are no long-range, well-designed studies looking at efficacy over time, but there has been some research on the treatment and the results are promising. In one study published in Neurological Research(2001), researchers reported that a spinal decompression therapy called VAX-D produced a success rate of 68.4%, compared with 0% for a placebo therapy in treatment of chronic low back pain. Another study demonstrated that a similar device called the Spina internal disc decompression unit or IDD had an 86% response rate. And a recent study from a team of researchers at the University of Illinois and Rome found a 71% success rate for treatment of herniated disk and other causes of low back pain using spinal decompression, with “success” defined as a reduction in pain to 0 or 1 on a scale of 0 to 5.

Dr. Wei noted that some people get much benefit from spinal decompression while others do not. He emphasized that it is not an option for everyone. “It’s contraindicated for a person who has metal implants in the spine or who has had a spinal fusion,” he warned. And it is not recommended for patients who have spinal instability or who have severe untreated osteoporosis. It’s better for people with a single-disk herniation than those who have multiple herniations. Also, people who are morbidly obese and/or who smoke probably won’t find much relief from spinal decompression.

The procedure is thought to be safe, though there is not a lot of hard science data supporting its efficacy. If you do decide to seek out this form of treatment, it’s safest and best to do so with the recommendation of your rheumatologist, who can help you determine whether it might work in your situation.

Police report drug haul seizure worth up to £30 million in Brownhills, England
June 25, 2021 · Uncategorized · (No comments)

Monday, December 2, 2013

Police in the West Midlands in England today said nearly 200 kilograms worth of drugs with value possibly as great as £30 million (about US$49 million or €36 million) has been seized from a unit in the town of Brownhills. In what an officer described as “one of the largest [seizures] in the force’s 39 year history”, West Midlands Police reported recovering six big cellophane-wrapped cardboard boxes containing cannabis, cocaine, and MDMA (“ecstasy”) in a police raid operation on the Maybrook Industrial Estate in the town on Wednesday.

The impact this seizure will have on drug dealing in the region and the UK as a whole cannot be underestimated

The seized boxes, which had been loaded onto five freight pallets, contained 120 one-kilogram bags of cannabis, 50 one-kilogram bags of MDMA, and five one-kilogram bricks of cocaine. In a press release, West Midlands Police described what happened after officers found the drugs as they were being unloaded in the operation. “When officers opened the boxes they discovered a deep layer of protective foam chips beneath which the drugs were carefully layered”, the force said. “All the drugs were wrapped in thick plastic bags taped closed with the cannabis vacuum packed to prevent its distinctive pungent aroma from drawing unwanted attention.” Police moved the drugs via forklift truck to a flatbed lorry to remove them.

Detective Sergeant Carl Russell of West Midlands Police’s Force CID said the seizure was the largest he had ever made in the 24 years he has been in West Midlands Police and one of the biggest seizures the force has made since its formation in 1974. “The impact this seizure will have on drug dealing in the region and the UK as a whole cannot be underestimated”, he said. “The drugs had almost certainly been packed to order ready for shipping within Britain but possibly even further afield. Our operation will have a national effect and we are working closely with a range of law enforcement agencies to identify those involved in this crime at whatever level.”

Expert testing on the drugs is ongoing. Estimates described as “conservative” suggest the value of the drugs amounts to £10 million (about US$16.4 million or €12 million), although they could be worth as much as £30 million, subject to purity tests, police said.

Police arrested three men at the unit on suspicion of supplying a controlled drug. The men, a 50-year-old from Brownhills, a 51-year-old from the Norton area of Stoke-on-Trent in Staffordshire, and one aged 53 from Brownhills, have been released on bail as police investigations to “hunt those responsible” continue. West Midlands Police told Wikinews no person has yet been charged in connection with the seizure. Supplying a controlled drug is an imprisonable offence in England, although length of jail sentences vary according to the class and quantity of drugs and the significance of offenders’ roles in committing the crime.

Parts of internet break as ‘512k day’ reached by routers
June 23, 2021 · Uncategorized · (No comments)

Thursday, August 14, 2014

On Tuesday many internet routers, used to find the pathways to different parts of the web, reached their memory limit of 512,000 entries in the tables they use to store the routes, causing problems for many users.

A router is used to direct a user towards the area of the internet where they will find the content they are looking for, e.g. a web page. The recent problem was caused by the arbitrary memory limit built into the design of many aging routers. A limit was needed to prevent the cost of hardware from becoming prohibitively expensive in the days when physical data storage was still comparatively expensive. This small amount of memory in turn limits the number of directions which can be stored on a single router leading to different parts of the internet. When this limit was reached, it caused outages of services among Internet Service Providers. Many routers, including older ones provided by Cisco Systems Inc., are limited to storing a total of 512,000 routes or paths.

This limit was reached on Tuesday, reported to have been triggered by Verizon publishing another 15,000 paths. Those affected included eBay, LastPass, and clients of the web hosting company Liquid Web, who lost much of their services until Verizon withdrew some of these new paths. Problems nevertheless continued throughout the day, even after the withdrawal.

A longer term fix is possible, but it would require manually replacing old routers with newer, more capable ones. It is hard to tell what issues would temporarily cascade though the internet by taking down routers from part of the internet for maintenance. Many experts have warned that problems could continue until these difficult fixes have been implemented, although the recent switch to a newer form of IP Addresses, IPv6, will temporarily help the issue. Wired News reported that Andre Toonk, a network engineer, had stated that the number of network outages on the internet, typically around 1,500, yesterday peaked at 2,587, enough to become clearly noticeable.

Wikinews interviews World Wide Web co-inventor Robert Cailliau

Wikinews interviews World Wide Web co-inventor Robert Cailliau
June 21, 2021 · Uncategorized · (No comments)

Thursday, August 16, 2007

The name Robert Cailliau may not ring a bell to the general public, but his invention is the reason why you are reading this: Dr. Cailliau together with his colleague Sir Tim Berners-Lee invented the World Wide Web, making the internet accessible so it could grow from an academic tool to a mass communication medium. Last January Dr. Cailliau retired from CERN, the European particle physics lab where the WWW emerged.

Wikinews offered the engineer a virtual beer from his native country Belgium, and conducted an e-mail interview with him (which started about three weeks ago) about the history and the future of the web and his life and work.

Wikinews: At the start of this interview, we would like to offer you a fresh pint on a terrace, but since this is an e-mail interview, we will limit ourselves to a virtual beer, which you can enjoy here.

Robert Cailliau: Yes, I myself once (at the 2nd international WWW Conference, Chicago) said that there is no such thing as a virtual beer: people will still want to sit together. Anyway, here we go.

China and Russia continue joint army exercise

China and Russia continue joint army exercise

Friday, August 19, 2005

Russian and Chinese armed forces began their first ever joint military exercises since the Korean War, when Russia was still part of the Soviet Union, on Thursday. The exercises are expected to last for eight days.

10,000 troops — about 1800 Russians, the rest Chinese — are participating in the war games. The exercises simulate United Nations-mandated police action to restore order in a fictitious country torn by ethnic unrest. They began in the eastern Russian port-city of Vladivostok and are supposed to culminate in a mock invasion involving beach landings and paratrooper drops off the coast of the Jiaodong peninsula in eastern China.

Russian and Chinese officials have framed the exercises as promoting international cooperation, but some in the US and elsewhere see the exercises as preparation for a possible future invasion of Taiwan, and as a challenge to US dominance in Asia — particularly in Afghanistan. The exercises come amid increasing speculation in the US that China may come to replace the US as the world’s greatest superpower.

Russia and China are working together to help form of a world order that will be based on multipolarity, respect for international law, and a leading role for the UN, Deputy Foreign Minister Alexander Alexeyev said on Monday, August 15, 2005.[1]

“The exercise will be carried out in the framework of the fight against international terrorism and extremism, to respond to new threats and challenges,” said Liang Guanglie, chief of staff of China’s armed forces.[2]

But others citing the use in the operation of advanced aircraft atypical of a peacekeeping operation, say the countries have ulterior motives. “The main target is the US. Both sides want to improve their position for bargaining in terms of security, politics and economics,” said Jin Canrong, professor of international relations at the People’s University of China.[3]

A Taiwanese official quoted by Mosnews.com said, China’s involvement in the operations represents “the biggest security threat in the Asiatic-Pacific region.” [4]

However other analysts speculate that Russia’s motivations for using the aircraft, including Tu-95 strategic bombers and Tu-22M long-range bombers, in the exercises may be to promote their sale to the Chinese.

“Military cooperation is linked with political and economic cooperation as part of a bigger package,” said Robert Karniol, Asia-Pacific editor for Jane’s Defence Weekly. “It’s not an adversarial posture.”[5]

Many have noted that the joint exercises are an emblem of increasingly warm relations between China and Russia since the dissolution of the Soviet Union. But writing In an editorial for the International Herald Tribune, Phillip Bowring disagrees, saying that the primary purpose of the operations is as a warning to the US. “The current Russia-China joint military exercises are not so much a symbol of trust and friendship between the two as a symptom of American overstretch. The two are reminding the United States of the limits of its unilateral global power.”[6]

Despite speculation into the motives of Russia and China, the US has given at least token support to the idea that the exercises could promote international interests. “We are following the exercises,” U.S. State Department spokesman Sean McCormack said this week. “We expect that they will be conducted in a manner that supports some mutual goal of regional stability shared by the United States, China and Russia.”[7]

Others say the operations are mainly a response to separatism and Islamic extremism in Russian Chechnya and the Chinese Xinjiang.

“The exercises are the logical continuation of the first signs of cooperation between Russia and China in the struggle against ‘orange revolutions,’ separatism and the dominant influence of the U.S. in the Euroasiatic sphere,” the Gazeta.ru news website wrote Thursday. Orange was the color adopted by supporters of last year’s revolution in Ukraine, which along with mass demonstrations in Georgia and Kyrgyzstan, brough pro-Western administrations to power. [8]

The war games have been called “Peace Mission 2005“.

Arkansas becomes first US state to criminalise treatment for transgender minority

Arkansas becomes first US state to criminalise treatment for transgender minority
June 16, 2021 · Uncategorized · (No comments)

Friday, April 9, 2021

On Tuesday, lawmakers in the Republican-governed US state of Arkansas became the first in the country to ban specific treatment types to transgender youth under the age of eighteen, irrespective of parental consent. It is to become law at the earliest by the 91st day after the legislative session’s recess, or July 30.

While vetoed by the state’s governor Asa Hutchinson on Monday, House Bill 1570, or the Save Adolescents From Experimentation Act was overturned 25–8 in the state Senate and 72–25 in the House of Representatives, prohibiting any healthcare professional from providing puberty blockers, cross-sex hormone therapy or gender-affirming surgery to anyone under the age of eighteen, as well as referring them to other providers.

Representative for Arkansas’ District 87 Robin Lundsturm said “[t]hey need to get to be 18 before they make those decisions”.

This ban also applies to those minors who are already receiving such treatments. The punishment will be a loss of medical licence.

While praised by conservative organisations such as The Family Council, Governor Hutchinson called the bill “a product of the cultural war in America”. In a statement Governor Hutchinson said, “I do hope my veto will cause my Republican colleagues across the country to resist the temptation to put the state in the middle of every decision made by parents and health care professionals”.

The American Academy of Pediatrics, representing 67 thousand paediatricians nationwide accused the bill of depriving young people of the medical care they need. Medical director at the University of California San Francisco Madeline Deutsch said of transitioning treatment “[t]here’s a general consensus among professionals in this field” that “[t]here’s tons of solid science supporting this approach”, and division head of adolescent and young adult medicine at a children’s hospital in Chicago Dr Robert Garofalo called it as “perpetuat[ing] the very things we know are harmful to trans youth”.

Executive director of the Arkansas American Civil Liberties Union (ACLU) Holly Dickson condemned the act’s proponents as “[disregarding] widespread, overwhelming, and bipartisan opposition to this bill and [continuing] their discriminatory crusade against trans youth […] to block trans youth from the care they need simply because of who they are is not only wrong, it’s also illegal, and we will be filing a lawsuit to challenge this law in court”. The institution is preparing for litigation; according to deputy director for transgender justice with the ACLU’s LGBTQ & HIV project Chase Strangio, “[w]e will always have your back and will be relentless in our defense of your rights.”

The bill’s passage comes after similar anti-transgender legislation in Arkansas and other states, including legislation signed by Hutchinson March 25 banning transgender girls and women from competing with their gender identity in sports, described by Dickson according to the AP as “discriminatory and shameful”.

After the lawmakers enacted this ban, pro-LGBTQ rights group the Human Rights Campaign said over 100 bills targeting transgender people have been filed, with bills of similar nature proposed in 20 states or more.
